Terms & Conditions of Contest

You must be eighteen years of age or older to have surgery. Surgery will be performed by Gary Kawesch, M.D in either our San Jose or Pleasanton location, and includes one year of follow up care. Surgical enhancements excluded. Drawings will be held monthly. Drawing is for bilateral laser eye surgery, using the Nidek EC-5000 excimer laser. Free procedure is for traditional LASIK using a microkeratome or PRK. IntraLASIK, Wavefront-guided LASIK, Wavefront-guided IntraLASIK or Wavefront-guided PRK is available, at an additional charge. Nearsighted patients wishing to have surgery with the Alcon laser may do so, at an additional charge. Winner will be notified via telephone or email. Procedure must be performed within three months of the drawing. Patient must be determined a candidate for laser eye surgery by Dr. Kawesch. We reserve the right to use each contest winner's name and photo for promotional use. Multiple entries in the same month will be discarded. Contest is run on several local area websites, from which only one winner is selected monthly.
